OFFICIAL RULES
Times/YMCA Healthy Living Contest
These rules are for the Times/YMCA Healthy Living Contest (“the Contest”), with entry into the Contest beginning Wednesday July 13, 2010 at 12:01 a.m. Eastern Time and ending Saturday August 20, 2011 at 11:59 pm Eastern Time. Entry into the contest constitutes the entrant’s full and unconditional agreement to and acceptance of these rules, terms and conditions.
ELIGIBILITY
NO PURCHASE IS NECESSARY TO ENTER OR WIN. Purchase will not improve chances of winning. The Contest is open to legal United States residents of Pinellas, Pasco, Hernando, Hillsborough, and Citrus counties who are 18 years of age or older at the time of entry, except that employees of the Times Publishing Company, its affiliates and subsidiaries, advertising and promotion agencies, contest prize suppliers, and participating advertisers and their immediate family members are not eligible to enter or win. “Immediate family members” means parents, siblings, spouses, children, grandparents and grandchildren, whether by current or past marriage, remarriage, adoption or cohabitation, and anyone living in the same household, whether related or not. Times Publishing Company’s determination as to eligibility is final and binding.
HOW TO ENTER
All entries must be received by August 20, 2011 by 11:59 pm Eastern Time. Only one entry per person is permitted during the contest.
There are two (2) ways to enter this Contest each week, as below:
(1) Enter online at tampabay.com/healthyliving
(2) Drop off a photo that reflects one of fifteen healthy living tips provided during the contest through either the St. Petersburg Times or tampabay.com/healthyliving along with your name, email address and phone number to one of the following YMCA locations.

All entries received by the deadline will be eligible for entry into an automated random
drawing. The Sponsor reserves the right to disqualify any entrant. Decisions of the Sponsor are final and binding.
The odds of winning depend on the number of eligible entries received.
The Times Publishing Company is not responsible for late, misdirected, ineligible, or any electronic malfunction or for any other problems or errors in transmission or delivery of entries.
All entries become the property of the Times Publishing Company. Each Entrant acknowledges and agrees that Sponsors may use information submitted to contact Entrant and/or to offer Sponsors’ products or services.
PRIZES
The following Contest prizes will be awarded:
There will be twenty-six (26) prize winners by random selection from entries submitted at each participating YMCA locations and one (1) winner selected from entries submitted to tampabay.com/healthyliving and one (1) Grand Prize Winner.
Each YMCA location and tampabay.com/healthyliving winner will receive a $50 YMCA gift certificate and the Grand Prize Winner will receive a one-year membership to the YMCA for up to four (4) persons, a 52-week subscription to the St. Petersburg Times and tickets to area attractions to include but not limited to: four (4) tickets to the Glazer Children’s Museum, four (4) tickets to Dinosaur World, four (4) tickets to Florida Aquarium, four (4) tickets to Lowry Park Zoo, four (4) annual passes to S. S. American Victory Mariners Museum, four (4) tickets to the Tampa Theatre, four (4) tickets to MOSI, four (4) tickets to Sunken Gardens, four (4) tickets to Splitsville and four (4) tickets to a Rays Game. Gift cards and tickets cannot be applied to previous purchases and are regulated by YMCA, Children’s Museum, Dinosaur World, Florida Aquarium, Lowry Park Zoo, S.S. American Victory Mariners Museum and Tampa Theatre terms and conditions. The gift certificates and annual YMCA membership prize is furnished by the YMCA. Area tickets are provided by individual attractions as listed above. The 52-wk subscription is provided by the St. Petersburg Times. The prize package is valued at $50 for each individual winner and up to $2,186 for the grand prize. The total value of all prizes is up to $3,536.
No alternative prize, cash or other substitution is permitted except in the event of prize unavailability. Sponsor reserves the right to substitute a prize or prizes of equal or greater value. Prizes are not transferable or assignable by the Winner.
All federal, state and local taxes, fees, and surcharges, and any necessary or appropriate insurance, are the responsibility of the Winner. The Winner agrees to pay all such taxes, fees, and surcharges. If requested, the Winner must complete a W-9 tax form provided by the Times Publishing Company prior to taking possession of the Prize.
Any transportation required for the Winner to take possession of the Prize is the Winner’s responsibility and will not be furnished by the Sponsor. Winner agrees to hold the Sponsor harmless from any and all liability for damages or losses of any nature incurred in connection with use of the Prize, including transit to and from any destination in connection with the Prize.
NOTIFICATION OF WINNER
Winner will be selected by August, 26, 2011 and notified by email and/or phone. If any Winner does not respond to the notification within twenty-four hours, or any Winner does not meet all eligibility requirements including signing the Affidavit and Release referred to below, an alternate Winner will be selected by random drawing. The Winner must produce adequate proof of identity, such as a driver license or photo I.D. card, in order to take possession of the Prize.
AFFIDAVIT AND RELEASE
The Winner will be required to sign and return an Affidavit of Eligibility and General Release, which includes an affirmation of eligibility, a release and waiver of legal claims, and consent to publicity. Failure to complete and sign the Affidavit of Eligibility and General Release will result in forfeiture of the prize and its award to another winner chosen by random drawing.

ADDITIONAL TERMS AND CONDITIONS
All federal, state, and local laws and regulations apply. Void where prohibited by law.
By entering the Contest, the entrant, for him/herself and for his/her heirs, executors, administrators, assigns and anyone else claiming by or through him/her, waives, covenants not to sue and fully releases and forever discharges any and all rights, claims or causes of action whatsoever that he/she may have against Sponsor arising directly or indirectly out of entrant’s participation in the Contest. For purposes of this waiver and release, “Sponsor” includes Times Publishing Company and its parents, subsidiaries, affiliates, officers, directors, agents and employees. This waiver and release covers, without limitation, any damage, liability, cost, attorneys’ fees and expenses whatsoever, whether known or unknown and whether foreseeable or not.
Sponsor reserves the right to make changes in the rules of the contest, including the substitution of a prize of equivalent value, which will become effective upon announcement.
Sponsor is not responsible for typographical or other errors in printing, the offering, or the administration of the contest or in the announcement of the prize.

The Sponsor of the Contest is Times Publishing Company, 490 First Avenue South, St. Petersburg FL 33701. Copies of the written contest rules are available online or will be sent by mail upon request if a self-addressed stamped envelope accompanies request.
